The deal is expected to be a loan deal until the end of this season with an obligation to sign Gabbiadini for £10.75m in the summer.
Then Sampdoria will pay a further £450,000 if the Italian striker reaches an agreed goals target this season and an additional £225,000 if they qualify for Europe.
Gabbiadini is expected to sign a four-and-a-half-year deal with Sampdoria and is set to arrive in Italy on Thursday, ahead of a medial on Friday.
The Southampton striker has previous experience with Sampdoria, playing for the Genoa club from 2013 to 2015.
Gabbiadini joined Saints from Napoli in January 2017 for £14.6m. The striker started life off in electric fashion, but has struggled with form recently.
